 SOLVING A CRISIS OF TRUST

A patient visits 10 different dentists and comes away with 10 different treatment plans. 

What ? How can this happen? Dentistry has its subjective and objective aspects. Let’s face it , the age of the dentist can play a role in treatment planning. Older doctors were trained to treat the caries by catching it early and subsequently restoring the tooth in a minimally invasive manner. Young dentists are being taught to watch the tooth or attempt to seal in caries or arrest the decay by way of silver diamine fluoride. Imagine the confusion for the patient. Who do they believe? A crisis of trust in our profession exists because of this dilemma.

You then must consider the variable of radiographic interpretation. One doctor may see the caries as an incipient lesion while the other may see it penetrate far enough into the dentin to require a restoration. We were all taught , and experience tells us, that the decay is typically farther advanced than what shows up radiographically. 

How about bone loss with respect to periodontal disease. In my general practice six point probing was performed yearly on every patient and more frequently on patients treated for the disease. Periodontal disease is site specific and episodic. A FMS must be taken and evaluated. Teeth must be probed to map out the disease. The reality is that only 30% of dental practices probe the entire mouth annually, falling short of performing the standard of care. Dentists must get on the same page and be consistent in their diagnoses and treatment plans. A dentist should not prescribe a routine prophy when a patient requires SRP. This prophy palace theory still exists today because some dentist don’t want to confront the patient who “ only wants what insurance will pay for”

AI COMES TO DENTISTRY

Enter artificial intelligence and the company Overjet. Founded in 2018 ,Overjet was developed by computer scientists from MIT and Harvard dentists. The company was founded by Wardah Inam, to raise the standard of care and enhance patient outcomes in dentistry. This technology can be more accurately described as augmented technology as it aids the practitioner rather than replacing them. This synergy yields a greater accuracy in detecting and quantifying pathology. Dental students at the University of Florida , improved their ability to detect decay by 40%. 

Overjet is the only company cleared by the FDA for detection to within a fraction of a millimeter. Its platform is used by most of the insurance companies in dentistry. According to company sources, Overjet is present in over 5000 offices in the US. It is available in foreign markets as well. 

CASE ACCEPTANCE RATES INCREASED BY 25%

I recently spent time reviewing the platform. It is without question the most transformative technology I have ever seen in dentistry. Private practice dentists will now be more accurate and consistent in their diagnosis.  Case presentation acceptance rates increased by 25% . Some dentists are better than others at communicating to patients their need for treatment. In dentistry, pathology often does not present with symptoms until there is a major problem requiring expensive treatment. Overjet puts the dentist, patient and insurance company on the same page. Finally we are moving dentistry to more consistent diagnoses and outcomes.

This platform as it exists will absolutely help with the communication with the payers. Remember it can be done while the patient is in the chair.

A GAME CHANGER

In my lecture titled The Periodontal Standard of Care for the General Dentist I talked about all the data required to arrive at a diagnosis. The way Overjet identifies and actually charts the calculus and bone level on the radiograph is a game changer. When I practiced general dentistry,  I routinely performed nonsurgical periodontal therapy. Any periodontist on the lecture circuit will tell you that 90% of periodontal treatment can be performed nonsurgically by the general dentist. My specialists appreciated this and reported that those patients I sent to them for surgery were pre sold because of my detail in data accumulation and patient communication. This was an arduous process documenting and communicating all of this information to the patient as well as the insurance company but it is absolutely required.
